  • Concurrent Enrollment Form

         University of Illinois students who want to take courses at Parkland College during the summer, fall or spring terms must download, carefully read, and fill out the appropriate Concurrent Enrollment form.  Effective SU12

  • Concurrent Second Degree
    This form is for students enrolled in other colleges on the University of Illinois campus wishing to complete major requirements for receiving an College of Agricultural, Consumer and Environmental Sciences degree. The form needs to be completed in full and submitted for review to the Academic Programs office. It is YOUR responsibility to obtain and submit with this form any documentation that supports the request. This form must be submitted to Academic Programs/128 Mumford Hall no later than 5:00 p.m. of the first day of classes one calendar year prior to the term of your graduation.
  • Continuing Student Status
    Required if you:
    • have completed a Bachelor of Science Degree at the University of Illinois at Urbana Champaign in the College of Agricultural, Consumer and Environmental Sciences and
    • are requesting to continue to take additional course work the subsequent semester immediately following the semester of graduation
  • Convocation- Request to Participate in May 2013 ACES Ceremony (August 2013 graduates only) 

         If you are a Bachelor’s student with 13 or fewer hours remaining and expect to complete your degree requirements in August 2013, you may be allowed to participate in the ACES Convocation ceremony and the Campuswide Commencement; however, your name will not appear in the Campuswide Commencement Program.

         Please complete this form in full, and submit it for review to the Student Development and Career Services office, 115 ACES Library.  It is your responsibility to obtain and submit with this form the documentation that supports your request.  This form must be submitted to the Student Development and Career Services Office, 115 ACES Library no later than 5:00 p.m. Monday, April 22, 2013. No late forms will be accepted.  Petitions will be reviewed after April 22, and an email notification will be sent to the email address you provide.

  • Credit/No-Credit
    The credit/no-credit grading option is designed to encourage student exploration into areas of academic interest that they might otherwise avoid for fear of poor grades.
  • Double Major
    This form is for students enrolled in the College of Agricultural, Consumer and Environmental Sciences wishing to complete major requirements for two majors within the College. The form must be completed in full and submitted for review to the College's Academic Programs office, 128 Mumford Hall no later than 5:00 p.m. of the tenth day of classes in the term of expected graduation.
  • Drop a Course after the Deadline
    This petition is required if you request to drop a course after the fourth week deadline for eight week courses (first half-and second half- sessions and after the eighth week deadline for sixteen week courses. In the case of extenuating circumstances, special administrative exceptions to the campus deadline may be made. The petition must include relevant documentation, as the request will not be reviewed/considered without these documents.
  • Graduating Senior Part Time Request
    Seniors with less than 12 hours left to complete their degree in the final semester may complete this form and submit to ACES Academic Programs.
  • Grade Replacement Form
    When a student repeats a course for grade replacement, the transcript is affected as follows: Both course enrollments and corresponding grades appear on the official transcript. The first course enrollment on the student's transcript will be permanently identified as a course that has been repeated for grade replacement. Where a course has been repeated for grade replacement, the course credit earned will be counted only once toward degree and program requirements. In the event that the student should fail the course on the second attempt, the student does not forfeit the original credit (§3-309 (a) (1)). (d) When a course has been repeated for grade replacement, only the grade received in the second enrollment for the course will count in the student's grade point average. In the event that the student should fail the course on the second attempt, while having earned a passing grade in the first attempt, the general policy governing course repeats applies: all grades received, passed, or failed will be counted in the graduation average (§3-309, (a) (2)) except as indicated in (§3-802) on minimum scholarship requirements for graduation.
  • Late Course Change Form
    This form is to be used when adding a course or changing sections after the deadline.
  • Overload
    • Fall/Spring request for enrollment in more than 18 hours; or
    • Summer request for enrollment in more than 9 hours.
  • Re-Entry
    This form is to be used for request to re-enter if you have only been away from the University for one calendar year or less and plan to re-enter into the same major and concentration. You must have left the University in good academic standing.

         Be aware that when you are not currently enrolled in U of I classes, you are not considered a continuing student and thus will receive an “open” time ticket. Open tickets allow registration after all priority ticket holders have been allowed to register. Thus, approved requests for reentry come with an open time ticket unless prior approval for exceptional circumstances has been granted by an assistant dean and approved by the Office of the Registrar.

  • Re-Entry (Long Form)
    Required if you:
    • have been away from the University for more than two consecutive semesters
    • left the University on drop status;
    • are applying to a different academic program;
    • were placed on 'must petition' status by their college; or
    • withdrew after the twelfth week of classes
  • Re-Evaluate Drop Status
    This petition can only be filed by students who were placed on drop status at the end of the previous semester.
  • Re-Entry: Non-Degree Status
    Required if you:
    • have completed a Bachelor of Science Degree at the University of Illinois at Urbana-Champaign and are requesting to take additional course work (not the semester immediately following graduation);
    • have previously been classified as non-degree and desire to take additional coursework in the College of Agricultural, Consumer and Environmental Sciences at non-degree status.
  • Re-Entry for a Second Degree
    Required if you:
    • have completed a bachelor's degree at the University of Illinois and
    • are seeking a second bachelor's degree in a College of Agricultural, Consumer and Environmental Sciences program.
  • Special Request
    This petition form is to be used for requests that do not fall under these categories: substitution of a required course; dropping a course after the campus deadline; and any type of re-entry request.
  • Substitution of a Required Course
    Petitions are to be submitted for all requests of substitutions for required courses. The College of Agricultural, Consumer and Environmental Sciences does not waive total hours required in a degree program. Requests to make a substitution for a required course should be made prior to enrolling in the alternative course; having completed the alternative course does not assure approval.
